THE VILLE ATHLETICS ATHLETE CODE OF CONDUCT

The Ville Athletics is committed to building disciplined, respectful, hardworking, and team-oriented athletes.

By participating in The Ville Athletics programs, the athlete agrees to uphold the following expectations:

RESPECT

I will show respect toward:
• coaches
• teammates
• officials
• opponents
• staff
• facilities
• equipment

at all times.

SPORTSMANSHIP

I will demonstrate positive sportsmanship during:
• practices
• games
• tournaments
• team events
• travel activities

I understand that poor sportsmanship reflects negatively on both myself and The Ville Athletics organization.

EFFORT AND ATTITUDE

I will:
• work hard
• remain coachable
• encourage teammates
• maintain a positive attitude
• represent The Ville Athletics with pride

SAFETY AND RULES

I will follow:
• team rules
• facility rules
• coach instructions
• safety procedures

at all times.

SOCIAL MEDIA EXPECTATIONS

I understand that inappropriate social media behavior may negatively impact:
• teammates
• coaches
• the organization
• sponsorship opportunities
• team culture

I will not post threatening, harmful, discriminatory, or inappropriate content related to The Ville Athletics.

ATTENDANCE AND COMMUNICATION

I understand the importance of:
• attendance
• punctuality
• communication
• commitment

and will communicate appropriately with coaches or staff if I cannot attend activities.

DISCIPLINARY ACTION

I understand that violations of this Code of Conduct may result in:
• warnings
• reduced participation
• suspension
• removal from activities
• removal from The Ville Athletics programs

depending on the severity of the behavior.

ACKNOWLEDGEMENT

I understand that The Ville Athletics is focused on athlete development both on and off the court/field and that my behavior contributes to the culture and reputation of the organization.

By signing this agreement, I acknowledge and agree to the following:

ASSUMPTION OF RISK

I understand that participation in athletics, sports training, practices, games, conditioning, camps, tournaments, travel, and related activities through The Ville Athletics involves inherent risks including, but not limited to:

• falls
• collisions
• physical contact
• equipment-related injuries
• sprains
• fractures
• illness
• permanent injury
• paralysis
• death

I voluntarily choose to allow the participant listed above to participate in these activities and fully assume all associated risks.

RELEASE OF LIABILITY

I hereby release, waive, discharge, and hold harmless The Ville Athletics, its directors, coaches, staff members, volunteers, affiliates, facilities, sponsors, and partners from any and all liability, claims, demands, actions, damages, losses, or expenses arising out of or related to participation in any The Ville Athletics activity.

MEDICAL AUTHORIZATION

In the event of an emergency, I authorize The Ville Athletics staff to obtain emergency medical treatment for the participant if I cannot be reached immediately.

I understand that I am financially responsible for any medical treatment provided.

HEALTH CONFIRMATION

I certify that the participant is physically capable of participating in athletic activities and that I have disclosed any relevant medical conditions, allergies, or limitations.

RULES AND CONDUCT

I understand that failure to follow team rules, coach instructions, safety guidelines, or appropriate conduct expectations may result in suspension or removal from participation without refund.

PHOTO AND VIDEO ACKNOWLEDGEMENT

I understand that The Ville Athletics may photograph or record activities during practices, games, tournaments, and events. Media usage permissions are addressed separately in the organization’s Media Release agreement.

ACKNOWLEDGEMENT

I have carefully read and fully understand this waiver and release agreement. I understand that by signing below, I am giving up certain legal rights.

THE VILLE ATHLETICS MEDIA RELEASE AGREEMENT

The Ville Athletics may occasionally photograph, record, livestream, or otherwise capture practices, games, tournaments, training sessions, camps, team events, and related activities for promotional, educational, marketing, and social media purposes.

By signing this agreement, I grant permission for The Ville Athletics to use the participant’s:

• video recordings
• audio recordings
• likeness
• name
• team participation images

in materials including but not limited to:

• social media posts
• website content
• promotional graphics
• advertisements
• highlight videos
• livestreams
• printed materials
• sponsor/promotional materials

I understand that:
• no compensation will be provided for use of these materials
• The Ville Athletics may edit or modify media for promotional purposes
• media may remain in use indefinitely unless otherwise required by law

If you DO NOT want the participant photographed or recorded for promotional purposes, please check the box below:

I understand that opting out may limit the participant’s appearance in team photos, promotional content, livestreams, or social media posts.

THE VILLE ATHLETICS PARENT / GUARDIAN CODE OF CONDUCT

The Ville Athletics is committed to creating a positive, respectful, safe, and development-focused environment for all athletes, families, coaches, officials, and staff.

By signing this agreement, I agree to the following expectations:

RESPECTFUL BEHAVIOR

I will treat:
• athletes
• coaches
• referees
• staff
• volunteers
• opposing teams
• spectators

with respect at all times.

SPORTSMANSHIP

I understand that positive sportsmanship is expected during all:
• practices
• games
• tournaments
• events
• team activities

I will encourage effort, teamwork, development, and respect over winning alone.

COACHING BOUNDARIES

I understand that coaching decisions, playing time, team assignments, and development strategies are determined by The Ville Athletics coaching staff.

I will not:
• confront coaches aggressively
• interrupt practices or games
• coach from the sidelines in a disruptive manner
• create conflict among athletes or families

SOCIAL MEDIA EXPECTATIONS

I will not post harmful, inappropriate, threatening, or defamatory comments regarding:
• athletes
• coaches
• officials
• families
• The Ville Athletics organization

on social media or public platforms.

SAFETY AND COMPLIANCE

I will follow all facility rules, organizational policies, and safety expectations established by The Ville Athletics.

DISCIPLINARY ACTION

I understand that violations of this Code of Conduct may result in:
• warnings
• suspension from events
• removal from facilities
• athlete suspension/removal
• termination of membership without refund in severe situations

ACKNOWLEDGEMENT

I understand that The Ville Athletics strives to provide a positive experience focused on athlete growth, accountability, discipline, teamwork, and character development.

THE VILLE ATHLETICS PAYMENT & REFUND POLICY

By registering with The Ville Athletics, the parent/guardian agrees to the following payment policies and financial terms.

REGISTRATION FEES

All registration/enrollment fees are:
• non-refundable
• non-transferable

The registration fee covers administrative setup, onboarding, operational costs, and athlete materials/spirit packs where applicable.

MONTHLY MEMBERSHIP PAYMENTS

Membership fees are billed on a recurring basis according to the selected program.

Current program pricing may include:

• Basketball Program — $159/month
• Soccer Program — $109/month
• Cross Country Program — $109/month
• Flag Football Program — $159/month

Pricing may change with advance notice.

PAYMENT RESPONSIBILITY

The parent/guardian is responsible for:
• maintaining valid payment information
• ensuring payments are completed on time
• updating expired or invalid payment methods

LATE OR FAILED PAYMENTS

The Ville Athletics reserves the right to:
• suspend participation
• restrict access to activities
• delay team placement
• remove athletes from participation

if payments become overdue or repeatedly fail.

REFUND POLICY

Refunds are generally not provided for:
• missed practices
• missed games
• voluntary withdrawal
• scheduling conflicts
• disciplinary removal
• injuries
• weather-related cancellations
• partial month participation

Refund requests may be reviewed on a case-by-case basis at the discretion of The Ville Athletics management.

CHARGEBACKS AND DISPUTES

Chargebacks or disputed payments may result in:
• suspension of membership
• collection actions
• removal from participation until resolved

MEMBERSHIP CANCELLATION

Membership cancellation requests must be submitted directly to The Ville Athletics administration.

Cancellation requests do not automatically cancel unpaid balances or previously scheduled charges.

ACKNOWLEDGEMENT

By signing below, I acknowledge that I have read, understood, and agreed to the payment and refund policies established by The Ville Athletics.
